While hiking in the hills, Addison and Sam chat about the time they almost dated in med school. Before things get interesting, there's a call for help. A man and his pregnant wife crashed over a cliff and the woman is still trapped inside the car. With no cell phone signal in the mountains, they are on their own. Oh, and one more thing. The woman's due date: Today.

The man, Jake, has a collapsed lung. Sam quickly improvises a chest tube that needs to be unclogged every half hour. He drags Jake closer to his wife in the car. The plan is for Addison to keep tabs on both of them while Sam goes for help. As Addison treats the woman, Kara, the car starts rolling down the mountain. Addison struggles to gain control of the runaway vehicle. It crashes into a downed tree.

With Addison now hurt and trapped, Sam can't leave. Without someone to unclog the chest tube, Jake will surely die. They are going to have to ride things out through the night. That's going be tough as Kara's water breaks and the baby is in a breech position. Sam now has no choice but to go for help in the dark of night.

Kara is panicked, putting both her and the baby in jeopardy. Jake yells support to her from afar and Addison tells Kara that she must fight to deliver this baby and survive. She has a husband who loves her. He'll be broken if she doesn't live through this. PUSH! The baby arrives. It's a girl! Addison yells out to Jake that he has a beautiful daughter. But Jake doesn't respond. Addison and Kara fear the worst, as do we.

Addison struggles to escape from the car but it's hopeless. Then a light streams down from above. Helicopters hover as Sam makes his way down to the area flanked by a team of rescue workers. He sees that Jake is still alive. As the rescue team works to free Kara from the car, she promises to name her baby Addison. Suddenly, Kara's wound ruptures. She's crashing. Then she's gone. Addison is devastated. It's beyond heartbreaking.

Pete and Sheldon literally trade war stories over a game of darts. No, really! When Sheldon tries to pick up a woman, Pete foils his flirting with some of his own. Sheldon accuses him of still being hung up on Violet which means he'll regret nights like this should she ever come around. Maybe he's right. When a woman tries to pick up Pete, he blows her off and sends her Sheldon's way.

A virgin bridegroom who wanted to rise to the occasion on his wedding night took a few too many pills and is now the not-so-proud owner of a never-ending erection. Good thing Oceanside Wellness has a new sexologist on staff to help settle this guy down.

The bride shows up and promptly heaves up whatever appetizer they served at the reception once she learns what Charlotte must do to get her husband swimming in lake flaccid. Violet discovers that the bride is pregnant. We did mention that the groom is a virgin, right? The bride blurts out the news of her impeding motherhood just as Charlotte works on her hubby's southern hemisphere. That type of news should be more than enough to deflate a man's...uh...confidence.

Violet accuses Charlotte of putting up a tough exterior because she's afraid of getting to know her patients. The conversation seems to strike a nerve, as Charlotte later tells the groom about how her first fairytale marriage ended because she bolted when things got hard. This convinces the groom to give his bride another chance.

An eavesdropping Violet is surprised to learn that Charlotte was married. She's even more shocked to learn that her story may have been one big lie. That lady is one complex carbohydrate. Later, she says to Cooper she has something to tell him. Hmm. Maybe it wasn't a lie.

Sam stops by to check on a still-hurting Addison. They pick up their conversation about what might have been had they hooked up in med school instead of marrying other people. They share a kiss. It's nice. But then Sam says, "You are no Naomi." She responds, "You're no Derek." They both start laughing. Yeah, having Addison and Sam in a relationship could make things a little awkward around Oceanside Wellness. And things are never awkward around that place, right?
