World-renowned heart transplant surgeon Dr. Alex Panttiere (Melissa George) operates on her own terms, whether it's at home - where she co-parents her two sons with her ex who left her for a man - or at work, where, as her hospital's chief innovations officer, she pushes the boundaries of medical science. As she cares for her patients, Alex must juggle a gaggle of interns, challenging co-workers and a by-the-books hospital chief operating officer (Shelley Conn). Life is further complicated by the return of Alex's former mentor, Dr. Jesse Shane (Don Hany). The one who got away, Jesse stirs old memories and deep feelings in Alex, which is problematic given that she is now involved with sexy liver transplant surgeon Dr. Pierce Harrison (Dave Annable), who is ready to take their relationship to the next level.

As chief innovations officer, Alex must meet with potential investors, and despite her trepidation about talking to businessmen, she is wildly successful at it, securing millions in funding for the hospital. This victory comes at a price when Alex learns the donor heart slated for her patient - a young woman in rapidly diminishing health who has been on the transplant list for months - has been reallocated to one of the wealthy patrons she just secured.

Undeterred, Alex searches for a new heart for her patient, and when the young woman's mentally unstable ex-boyfriend kills himself so that his former flame can have his heart, Alex is determined to fulfill his wish, despite the tragic circumstances. Using an experimental procedure without waiting for hospital committee approval, Alex is able to save her patient's life. It's a risky move, but it's all in a day's work for Alex, who, having lost someone close to her, will do whatever it takes to save a life.
