The 33-year-old model, who was battling substance and alcohol abuse, was found dead at her home in Austin, Texas in mid-April after a friend requested a wellness check from police.

AceShowbiz - Playboy Playmate Ashley Mattingly chose to end her life gruesomely. Nearly two months after she was found dead from suicide, the ex-girlfriend of "Prison Break" actor Lane Garrison was uncovered to have passed away from a gunshot to the head.

An autopsy report from the Travis County Medical Examiner's Office, per reported by TMZ, revealed that the 33-year-old model "put a handgun in her mouth" before pulling the trigger. Her toxicology reports additionally disclosed that she had opiates and benzos in her system at time of her passing.

Mattingly died from a suicide on April 15. Her body was found the next day at her home in Travis County outside of Austin, Texas. Her twin siblings, William and Christy Mattingly, claimed that she left a suicide note, but they were not yet able to read it. They further stated that a friend asked police to perform a wellness check on their sister since they could not reach her.

In their statement about Ashley's death, William and Christy told PEOPLE, "We learned late Thursday night that it is believed that our dear sister with a larger-than-life personality took her own life." They added, "It's maybe no secret that Ashley struggled with alcohol and substance abuse, but she wanted to and was working toward getting better."

"We are now loving on her sweet golden retriever puppy that she recently adopted to overcome these demons," they went on sharing. "She will continue to live on through nieces whom she loved dearly and always thought of no matter how far away she was, a twin brother, two sisters and numerous other friends and family members. We will forever cherish her memories and know that her joy is eternally sealed."

At the end of their statement, the siblings expressed hope that Ashley's death could bring awareness to suicide prevention. "Our hope is that Ashley's story can continue to shed a light on suicide awareness and prevention," they concluded. "We love you Ashley."

Ashley made an appearance on the adult magazine as 2011 Miss March. In 2012, she made headlines for being a victim of domestic violence after an altercation with her then-boyfriend, Garrison, turned physical, resulting her getting a slap in the face in the lobby of her Beverly Hills apartment and him arrested for misdemeanor battery. Four years later, it was her who got arrested for DUI suspicion.
