Days after announcing her tragic death, the family of the season five star of the MTV show sets up a GoFundMe account to cover the costs of her memorial and help support her 6-month-old daughter Lyla's 'future care.'

AceShowbiz - The cause of death of Jordan Cashmyer has finally been unveiled. According to the "16 and Pregnant" alum's family, the former reality star passed away after she lost her battle with addiction.

A little over a week after Jordan's father, Dennis M. Cashmyer Jr., confirmed the sudden death of the season five star of the MTV reality show, her family set up a GoFundMe account to help support the late reality star's "sweet little 6-month-old girl" named Lyla. In it, the family stated, "Addiction plagues many families, and our family was not immune to it."

In the description of the fundraising page, Jordan's family remembered the late TV personality as someone who "was always so full of life and full of personality." The note added, "She had a way of always making those around her laugh and smile, and her smile was truly contagious."

"Our family is heartbroken, and we will forever miss Jordan," the family penned. "We are now faced with the grim outcome that our grandaughter that we have been raising will never get to make memories with her mother or father, never getting to know them."

The note went on to say that the baby girl's father died about four months before Jordan. "Lyla lost her father [Jordan's fiance Michael Frank Schaffer] to addiction at two months & now her mother at six months," the statement read. "Our family has to now not only put our daughter to rest but navigate through starting over with an infant who has lost both her parents in her six months of life."

"Please help our family cover the costs of Jordan's memorial and start a fund for Lyla's future. We did not foresee starting back over, and all that comes along with that," the family asked for help. "Lyla has been our family's greatest joy and addition; she has been our saving grace during this difficult time." The statement concluded, "We pray Jordan is finally at peace. Lots of Love, The Cashmyers."

Jordan's father, Dennis, announced the former reality star's tragic passing on January 16 in an emotional Facebook post. Alongside a black-and-white photo of the late TV personality, he wrote, "Last night I received a call no parent ever wants. My oldest beloved daughter, Jordan has passed away, she was only 26 years old."

Jordan once attempted suicide in 2014 after her split from her then-boyfriend Derek Taylor following their daughter Genevieve "Evie" Shae Taylor's arrival. Then in 2016, she admitted herself to a rehab facility at the age of 21 after she injured herself in a drunken stupor. A source claimed that the incident, which left Jordan with a concussion and black eye, made her think that it was time to "get her life together."

A year later, Jordan was arrested twice for drug possession. At the time, she was charged with one count of possession of marijuana and three counts of possession of drug paraphernalia. In 2019, she was also taken into police custody on drug charges.
