Twins Kelly and Derek (18) are working as lifeguards at the beach when a couple who were thrown from their boat wash ashore. They managed to rescue them, but then the unmanned boat comes crashing into the pier injuring Kelly and Derek. Kelly was the most severely injured and dies shortly after she arrives at MT1; though Dr. C fixes Derek's neck wound, when Derek finds out about his twin, he develops broken heart syndrome. Carla, the woman thrown from the boat, is paralysed and it appears permanent. Turns out Carla was vacationing with her new boyfriend, who was thrown from the boat but relatively unharmed. Not knowing Carla was separated from her husband, the hospital calls him. Thinking that the paralysis is permanent, the boyfriend abandons her, but her husband stays, however, it turns out she's going to be fine. Also, Rick Deleo, Dr. C's wayward brother, is the rescue helicopter pilot who brought in the red bands. He seems to be flirting with Zambrano, which drives Dr. C crazy, however, he's not -- he's had some medical problems and wants a second opinion. Zambrano diagnoses kidney cancer and urges Rick to tell Dr. C.
