In the Season 3 premiere, Detective Antonio Dawson (Jon Seda) races to the top of an apartment building to stop a jumper, who is desperate after discovering that her boyfriend is cheating on her. The boyfriend in question - Benji Keyes - is the brother of Chicago's most wanted heroin kingpin, Derek Keyes. With an assurance that he will protect her by putting both brothers in jail, Dawson talks the woman down from the ledge.

Back at District 21, detectives Jay Halstead (Jesse Lee Soffer), Kevin Atwater (LaRoyce Hawkins), Alvin Olinsky (Elias Koteas) and Officer Adam Ruzek draw up plans for bringing Derek Keyes to justice. The key to the plan, everyone realizes, is grabbing Benji without anyone - especially his brother - finding out about the extraction. As the plan is finalized, Halstead reaches out to Sergeant Hank Voight (Jason Beghe) about troubled detective Erin Lindsay (Sophia Bush). Lindsay, struggling with alcohol and guilt over putting her friends and colleagues in danger, is technically on an approved furlough from the squad. Voight sidesteps Halstead's request to contact her, insisting, "She made her choice."

In a daring covert operation under cloak of darkness, the team infiltrates Benji's heavily guarded brownstone, taking him without incident... except one, as an unaware young man steps out of Benji's bathroom. As a witness to the operation, he is also taken into custody.

At District 21 later that night (and with the 10 kilos of heroin seized in the operation on the table), Voight makes a deal: in exchange for information leading to his brother's apprehension, Benji and the young man - in actuality, his boyfriend - will walk.

The next morning, Desk Sergeant Trudy Platt (Amy Morton) calls Olinsky to her desk. It seems a young girl, Michelle Sovana, from juvenile detention, keeps calling for him. Not only that, but she's been calling for weeks. Olinsky confides in Platt: the girl is actually his daughter from a woman he slept with years ago when he was deep undercover. The mother is about to begin serving a four-year jail sentence, and Olinsky doesn't know how to help his daughter. Platt says not to worry - she has a friend in juvenile detention, and she'll take care of it. Just like... she'll take care of the wedding plans for Burgess and Roman.

Halstead finds Lindsay outside a nightclub and confronts her in the morning after a night of heavy partying. He reminds her that her three-week furlough with Voight ends the next day and implores her to return to work. He tells Lindsay that he needs her, as she made him a better cop.

Under the team's watchful eye, Benji arranges a meeting with his brother at Midway Airport. The ever-suspicious kingpin changes the plans, forcing the team to improvise on the extraction point. With Halstead posing as an undercover buyer, he and Benji finally meet Derek outside a private jet area. Sensing a trap, Derek kills Benji and takes Halstead hostage - and despite giving chase, the team is unable to stop him. That evening, Derek gives the team an ultimatum: turn over all of Intelligence's confidential informant files by morning, or Halstead dies. And, Derek says, he wants a lone female officer to serve as the go-between. With Halstead's life on the line, and knowing they can't turn over the actual files to Keyes, the team gets to work creating false documents.

Meanwhile at her apartment, Lindsay has a moment of clarity as she confronts her mother, Bunny. A phone call from Alvin at District 21 leaves her dazed when he informs her that Halstead has been kidnapped.

Voight reaches out to a man from his past at Stateville Prison for information on Derek's current whereabouts. The prisoner - Eddie Little - agrees to assist Voight, in exchange for help when he hits the street in four months. Deal.

Back at the precinct, the team debates over who will serve as the "lone female officer" Derek demanded. To everyone's surprise, Lindsay shows up to volunteer. Initially hesitant, Voight agrees... on the condition Lindsay understands this is a one-shot deal - and not a reinstatement. She agrees and is prepped for the operation. With fake files in hand and the team watching her every move, Lindsay shows up at a predetermined meeting spot for the exchange. But as Derek has shown, he's full of surprises - and promptly abducts Lindsay as well.

With time running out and two of his team members in jeopardy, Voight tracks down Derek's location using information from Eddie, his prison informant. Just as Derek determines that the files are fake and prepares to kill Halstead and Lindsay, Lindsay breaks free and is forced to kill Derek. The team arrives and quickly escorts a badly beaten Halstead to the hospital.

Meanwhile, Olinsky meets his long-lost daughter as she's released from juvenile detention. The reunion is short-lived, however, as she just as quickly departs with a friend in a waiting car...

As the dust settles from the operation, Lindsay pleads with Voight to reinstate her. But she discovers getting her badge back comes with conditions, including severing all ties with her drug-addled mother.

The team meets at the bar that night, in part to discuss the upcoming wedding of officers Sean Roman (Brian Geraghty) and Kim Burgess (Marina Squerciati). But as Roman steps out for a second, Burgess makes a startling discovery among the items that have fallen out of her fiance's bag: a hypodermic needle. The truth is never what you think.
