Catherine is agitated and refuses Dr. Hartramph's urging to be admitted into a hospital. Her behavior is becoming increasingly erratic. Hartramph can't see her anymore if she refuses to let her help. So Catherine bids adieu to her longtime therapist.
Mira is a housewife who confronts a knife-wielding man on the street. The team wonders if it's really a display of bravery or if she has an absence of fear. Farrah wants a consult, but Catherine blows her off. She claims it's because she prepping for a speech at Harvard. As for Mira, her condition briefly puts her young son in grave danger.
Catherine asks Farrah to be her new psychiatrist. Doctor-patient confidentiality would apply. Later, she asks to spend the night with Reagan and Esme after suffering hallucinations. Catherine has severe insomnia. She keeps checking the window fearing someone is out there. The next morning, she goes to see Will. She's in an incredibly manic state, but calms when Will promises he'll never tell Esme that she's her mother.
Bickman is reluctant to go back into the operating room after Joey's botched surgery. Catherine encourages him to see Lina so that she can re-wire his brain. It doesn't work. However, Bickman overcomes his fear of operating when there's no one else around who can save a patient. In other news, Leo scores incredibly high on his Step 2 exam. He lets Lina's ex, Bruce, in on her agoraphobia secret. The understanding plumber refuses to let Lina push him out of her life.
Farrah figures out that Catherine is bipolar. Her meds aren't currently working. Catherine agrees to go into in-patient treatment immediately. Then she throws her prescription of Oxy in a backpack. She blows off Reagan's request to take her to a psychiatrist claiming she's already left for Harvard. Catherine is out of control as she drives into the night. She pulls off the road, strips down and walks into the woods.
