Creator Bruce Miller discussed the careful consideration given to how much trauma is shown onscreen, emphasizing the importance of not overwhelming viewers.

Miller revealed in an interview with The Hollywood Reporter on April 8 that the series offers limited glimpses into Aunt Lydia’s (played by Ann Dowd) past. The production team was "very mindful" about the trauma level they exposed the audience to, making sure these moments aligned with the narrative goal of explaining Lydia’s mindset at the dawn of Gilead and the reasons behind her decisions.

The series is based on Margaret Atwood’s novel of the same name and serves as a continuation of the dystopian world first introduced in Hulu’s The Handmaid's Tale, which aired from 2017 to 2025. The original story depicted a grim future where declining fertility leads to women being assigned reproductive roles under a totalitarian regime.

The Testaments is set four years after the events of The Handmaid’s Tale. It follows Aunt Lydia as the narrator and introduces new characters Agnes and Daisy, who infiltrate Canada disguised as "Pearl Girls" to smuggle critical evidence against Gilead.
