Juliet tries to wake up Jack, but there's something wrong with him. He doesn't seem well. He stumbles out of his tent as a loud argument ensues on the beach. A group of angry survivors are going at Faraday and Charlotte. Jack tries to calm everyone down. He assures them they will be rescued, but suddenly he collapses.

WHOOSH to Jack in a nice, comfy bed when the phone rings and wakes him up. It's his assistant reminding him of his schedule changes. Jack gets up and goes into the bathroom. A woman in the shower tells him she bought him a new razor. She's not a fan of his growing beard. The shower stops and out steps Kate. Jack hands her a towel and they kiss. It's the future, and Jack and Kate are living together.

WHOOSH to the island where Sawyer, Claire, Aaron and Miles trek through the jungle on their way back to the beach camp when Miles suddenly stops. He asks them who Danielle and Karl are because he's standing on top of them. Miles drops down and starts digging, uncovering their bodies. Sawyer asks if Miles' buddies killed them, but Miles says he didn't sign up for this.

At the beach, Juliet talks to Jack. He thinks it's food poisoning. Juliet disagrees. She makes him lift his shirt, and she presses on his side. Jack grimaces in pain. It's his appendix and it hasn't ruptured. Yet. Juliet considers it and states the simple, pragmatic reality. They have to take it out.

Juliet asks Sun to go to the medical station for equipment, but Sun doesn't know what the supplies look like. Faraday knows what the equipment is, so he offers to go with her. Jin and Charlotte go, too. Kate asks why they don't just take Jack to the medical station, and Juliet tells her if they move him his appendix might rupture. If that happens, Jack will die.

WHOOSH to Jack reading Alice's Adventures in Wonderland to Aaron as he falls asleep. Jack tucks Aaron in and quietly shuts the door. Kate stands in the hallway smiling. Jack's a natural. She likes seeing him with Aaron. She's glad he changed his mind. She's glad he's there. Jack picks Kate up and takes her into the bedroom.

WHOOSH to the beach where Bernard helps prepare a surgery table. Rose is concerned why Jack got sick. Bernard says it's just bad luck, but Rose isn't convinced. The day before they're supposed to be rescued, the guy they count on suddenly comes down with a life threatening condition? Bernard says people get sick. Not here, Rose reminds him. Here they get better.

As Juliet preps Jack for the surgery, Jack says he wants to talk her through it. He doesn't want to be unconscious. They can numb the area with lidocaine and Kate can hold a mirror up so he can watch. Juliet suggests Bernard since he's a dentist and has medical experience, but Jack insists on it being Kate. Juliet says okay.

WHOOSH to Jack in his doctor's offices as he walks a patient out. Another doctor comes up and asks for his opinion on an x-ray, and Jack catches a glimpse of a man sitting in the reception area who looks a lot like Christian Shephard, Jack's father. But Christian is dead. Before Jack can get a second look, the man is out the door and gone. Then the receptionist gets Jack's attention. A doctor from Santa Rosa needs to talk to him.

At Santa Rose, Dr. Stillman tells Jack that Hurley refuses to take his medication, has stopped sleeping and doesn't think that Dr. Stillman exists. Jack talks to Hurley, who tells him they're all dead. All the Oceanic Six are dead and never got off the island. He says Charlie comes and visits him. Charlie told Hurley yesterday that Jack was going to come by and wanted Hurley to give him a message. "You're not supposed to raise him, Jack." Jack has had enough of this. He tells Hurley to take his meds and leaves. Hurley tells Jack that Charlie also said someone was gonna visit him, too. Soon.

Jack sits in his car outside Santa Rosa, thinking about what Hurley told him. He finally drives home. He wakes up Kate and tells her he had to stop and pick up something. Kate sees something is bothering him and asks what's wrong. Jack pulls out a diamond ring and asks Kate to marry him. Kate's stunned. She says yes, of course she'll marry him. She kisses him, but there's still an anxious look on Jack's face.

WHOOSH to the island. Sun, Jin, Faraday and Charlotte reach the medical station. Faraday offers to go in first to make sure it's safe, and Charlotte is appreciative. Jin asks Sun in Korean if Charlotte realizes Faraday likes her, and Sun says of course she knows. She's a woman. Charlotte smiles and Jin sees it. Wait, does Charlotte understand Korean? Sun leads them into the secret room and Faraday finds the supplies and equipment needed. Sun talks to Jin in Korean and tells him that Faraday and Charlotte aren't going to help them. Jin tells her not to worry. He promises he will get her and their baby off the island. Then Jin catches Charlotte listening, almost like she's eavesdropping.

As Sawyer, Claire and Miles continue through the jungle, they hear someone running through the brush. Sawyer aims his rifle when Lapidus breaks through the tree line. He's surprised to see them but warns them they have to hide. Immediately. They hide in the brush as Keamy and his men break into the clearing. Lapidus tells him the chopper is a click away and leads them out of there.

At the beach camp as Sun, Jin, Faraday and Charlotte return with the supplies. Juliet asks Sun to help her sterilize the instruments, and they head over to the kitchen area. Jin stops Charlotte and speaks to her privately... and in Korean. Charlotte pretends to not know what he's saying, but Jin knows she understands him. He warns her that if she continues to lie, he'll hurt Faraday. He'll break his fingers one at a time. Charlotte stares at him and responds in flawless Korean. What does Jin want? When the helicopter comes back, Jin wants Charlotte to make sure Sun gets off the island. Charlotte asks about him and the rest of his friends. Jin doesn't care about them or himself, he just wants to get Sun off the island.

With Bernard and Kate beside her, Juliet begins the surgery. She makes the incision, and even though the area has been numbed, Jack still writhes in pain. Juliet warns him he has to stop moving around. Jack complains he can't see what she's doing in the mirror, and Juliet has had enough. She orders Bernard to knock Jack out and Kate to leave. Jack protests but Bernard agrees with Juliet. He pours some chloroform onto a rag and presses it over Jack's face.

WHOOSH to Jack's office. He's working late at night when the smoke detector goes off. Jack goes out and pulls the battery out when someone calls him name. Startled, Jack turns and sees Christian Shephard sitting in the waiting area. Then a co-worker comes around the corner and asks if everything is okay. Jack jumps, thinking he was the only one there. When he looks over, the chair is empty. He asks her if she could write a prescription for clonazepam. He says he's been having a lot of nightmares and trouble sleeping. At home, he overhears Kate talking on the phone to someone, and it seems like she is covertly planning something. When she realizes he's there, she gets off the phone and claims the call was nothing. Jack isn't convinced.

WHOOSH to the beach camp, Juliet tells Kate everything went well. Juliet confesses that Jack kissed her. She reassures Kate that it was Jack simply proving something: that he doesn't love someone else. The implication is clear, and Kate thanks Juliet for saving Jack's life. Kate leaves, and Juliet tells Jack that she knows he's awake. Jack heard every word she said to Kate.

WHOOSH to Kate's house. Kate comes home, and is surprised to see Jack there. He looks upset, and she asks what's wrong. Jack asks her where she was. Kate says he's just going to have to trust her and leave it be. But Jack can't do that. He asks her again where she was. He heard her on the phone the other night. Who was she talking to? Kate tells him to stop, to let it go. But Jack wants to know. Finally, Kate says she was doing something for Sawyer. She made him a promise. Jack is hurt. Sawyer's not there. He made his choice and chose to stay. Jack is the one who is there. He's the one that saved her. Kate tells him he can't do this. She can't have him around her son. Jack tells her she's not even related to him. Then Kate notices Aaron in the hallway, watching them fight, crying. Kate pushes past Jack and picks him up, telling him everything is okay. But Jack turns and walks out.

WHOOSH to the island where Sawyer, Claire and Miles are sleeping. Claire hears something and wakes up. Across the fire she sees someone holding Aaron. It's her dad, Christian Shephard. When Sawyer wakes up, he sees that Claire is gone. Miles tells him she had a visitor last night and left with him. She called him "dad." Sawyer hears the faint sounds of Aaron crying and takes off running through the jungle. He finds Aaron wrapped in a blanket at the base of a tree, and he's all alone. Sawyer yells out for Claire, but no one is around.
