Fleeing from an arranged marriage, Neela's sexy cousin Jaspreet has come to stay, with plans to debauch herself. She convinces a reluctant Neela to bring her to work. Sam and Ethan join Skye for early morning triathlete practice, which leaves Sam heaving and limping. Bettina pumps a nervous Pratt up for an interview with Anspaugh for the Chief job. Gates meets an ambulance containing Beth Ackerman, who fell down an escalator at Bloomingdale's, injuring her ankle. Beth also complains of abdominal pain.

Pratt pitches his resume to Anspaugh and Dr. Everett Daniels, but Anspaugh doesn't see him in the Chief's job. What has Pratt done to prove his leadership skills? With that being said, Pratt gives his two week's notice, preferring to work someplace where he's respected. Morris thinks Pratt is overreacting; after all the ER is his family. Pratt turns his attention to 8-year-old Carlos Moore, who passed out and fell down in gym class, injuring his head. While stitching him up, Pratt learns that Carlos has HIV.

Beth is a solider on leave from Iraq, and she was out celebrating last night with her boyfriend Jack, a marine stationed in Guam. He's elated to discover that Beth is pregnant, but Beth looks dubious when an ultrasound indicates that she's at eight weeks. Jaspreet is turning rounds upside down, taking photos of the doctors. Crenshaw asks Harold to give Jaspreet a tour, prompting Harold to have an anxiety/asthma attack, which he claims is due to psychosexual stress.

Jeanie Boulet is counseling HIV+ teens at her South Drexel Clinic when she receives the call that her son Carlos is in the ER. She barges past Morris into the ER, where only Chuny remembers her, and refers her to Pratt, who wants Carlos to have a CT scan. Jeanie means no disrespect and asks to speak to the attending, since her son's case is complicated. Pratt tells her that he is the attending. Tension brews as Neela attempts to realign Beth's foot in order to save it. When Grossman, head of the ortho team arrives, his scolding turns to admiration when the foot is saved.

Although Jeanie prefers Carlos to have an MRI, Bettina talks her into the CT scan for Carlos. Jack demands to know the father of Beth's baby, since it couldn't have been him. She admits that she was raped by her sergeant, but Jack thinks she's lying, since he just saw them laughing together. Jack scuffles with Gates and slams out. Pratt calls his connection at Northwestern for a job. Morris zeroes in on Jaspreet, stealing her away from Harold.

Beth tells Skye and Sam about the rape. At first she thought they were being attacked, but when her sergeant put his hand over her mouth, she obeyed his command to be quiet. She didn't tell anyone because she needed the other guys in her squad to watch her back. Sam tells her that the sergeant should be punished. Sam's been through it, and the only things to be gained by keeping quiet are nightmares and panic attacks.

Haleh and Jeanie reminisce about old times. Jeanie tells Haleh that she and Carlos' adopted father, Reggie, separated two years ago. Skye offers to expedite the results of the CT scan. Pratt meets Daniels in the Jumbo Mart for coffee. Pratt claims that the head of the department at Northwestern is looking to mentor his replacement, and they have an interview tomorrow. Daniels asks Pratt if he's the kind of fighter that gives up after being knocked down in the first round, and advises him to think about it.

Pratt returns to the ER, angry to find Skye getting off the elevator with Carlos' CT scans. Skye wants to talk with Pratt, but Jeanie demands to know the results right away. Carlos has a lesion, which Jeanie concludes means he has full-blown AIDS. Later, Pratt lays into Skye for butting in with his patient. She claims that they are supposed to be working as a team, but Pratt thinks she's a micromanager in denial.

Jeanie breaks down in front of Haleh, who tells her to wait to find out more information about the lesion before reaching any conclusions. Pratt tells Jeanie the best way to investigate the tumor is with a needle biopsy, no need for surgery. Outside, Neela finds Harold letting off steam playing four square with kids from the waiting room. He's agitated because Morris stole Jaspreet. When Morris and Jaspreet pass by, Harold arranges for a double date with Neela later that night.

Reggie and Jeanie argue about telling Carlos that he has AIDS. Jeanie wants to take it one step at a time, which she doesn't think is lying. Reggie thinks Carlos already knows, having lived around Jeanie's clinic, but acknowledges that Jeannie is incredibly stubborn. Skye tells Sam that she is sorry to hear that she was raped, but she doesn't think it's appropriate to discuss personal matters with patients. Jack runs into Gates in the Jumbo Mart. Gates tells him that if he wants to be a soldier, Jack should start by defending Beth.

Reggie stands by as Jeanie tells Carlos that he has a spot, and they need to take a biopsy. Carlos wants to know if he has a tumor. Jeanie insists that they will get through it together, as Carlos starts having a seizure. Skye and Jeanie argue with Pratt over how to proceed, but he intubates Carlos, stabilizing him for surgery. Gates and Sam visit Beth, surprised to see that Jack has come back. Gates offers a D and C, but Beth insists that she's going to keep the baby, so she doesn't have to return back to Iraq.

Things are tense at the bar on the double date, with Harold trying to throw egg on Morris' face in front of Jaspreet. Grossman calls to Neela. He's arranged for her to do an ortho rotation with his team. Next week she'll be hanging with the "cool kids," and she better get ready, because they're pretty tough. Gates tells Beth that having the baby isn't the answer - what happens if the sergeant rapes someone else? Jack promises to support Beth. He won't let anything else bad happen to her, including going back to Iraq.

Pratt finds Reggie sitting outside the biopsy theater. He can't watch, but Jeanie can. Overhearing Ethan ask Sam for a date, Gates tells her that she was in his dream, begging to do his laundry. What could it mean? Pratt and Jeanie watch Carlos' biopsy. She wonders if everything that she has been fighting for is a crock, after watching people close to her die. Pratt tells her to keep doing what she's doing, and Jeanie admits that he's not so bad. Afterwards, Pratt calls Daniels to tell him that he's ready for round two.
