Sam and Kovac were bringing in an elderly patient who was coming from his stay at a hospice. Mr. Gower brought with him a photographer who continued to take pictures of him, even after Sam pleaded with her to stop.

Sam asked the nosy photographer, Diana, to stay out of her way as Mr. Gower recuperated. Diana stated that she was employed by the man to follow his life in the hospice and record it with photographs until his death. Diana told Sam that she had covered death most of her career and that she was used to it by now.

The ER became chaotic when a father and son were brought in after a car accident. The father recognized the patient in the next room as the man who caused the accident and wanted a confrontation. He shouted at the man, blaming him for killing his wife and daughter and putting his son in the ER.

Hope tried to show Abby the binder she had of all her wedding ideas. Abby was too busy to deal with it, so Hope foolishly left the book out and Sam began to look through it. Abby became frustrated with Hope's irresponsibility with almost letting out the secret of Kovac and Abby's engagement.

Abby and Kovac found some alone time to discuss their hush-hush wedding plans. Kovac broke the news to Abby that Morris knew their secret and Abby broke the news that Hope found out, too. They both laughed at the thought of just running off to Las Vegas and eloping.

One more person came into the ER from the traumatic car accident. It was the wife of the man who had caused the accident. Things became tense when the surgical intern began to take all the credit for Neela's work when Dubenko entered the operating room. Ray tried to defend Neela's work, but Dubenko cut him off and asked Neela to leave the OR.

Sam found some alone time with Diana's computer and began to look at some of her work. Diana watched in the background as Sam sifted through photographs. Sam was looking at a photograph of Diana after a round of chemotherapy. Diana explained she had ovarian cancer and was on her third treatment with it. Diana said her cancer was a wake-up call to start living. She gave Sam her expensive camera as a birthday present.

Dubenko had asked his surgical intern to do a simple task while in surgery and she kept messing up. Finally, he had to step in and do things himself. Neela watched in the background as Dubenko was getting more and more frustrated with how things were going. The woman died on the operating table as her husband was in the next room watching on.

Neela confronted Dubenko about his hostility towards her. He said that he felt bad that he let their personal friendship get in the way and he had to be tougher on her. Dubenko defended his treatment of Neela and then confessed to having feelings for her, which is why he had to make sure there was a distance kept between them.

Neela and Ray let the husband say good-bye to his dead wife. He told the doctors the accident was caused when he tried to read directions while his wife was asleep in the passenger seat. Neela couldn't believe he graduated college and was illiterate. He told Neela that nobody cared because he was so good in sports, so he just got passed along.

Gates struggled with how he was going to break the news to Sarah that he might be her biological father. After dinner, he told her the story of her mother's confession. Sarah was angry that her own mother never told her. Gates told Sarah he wanted to take a paternity test. Sarah accused him of making up excuses to get rid of her and refused the test.

Kovac and Abby slipped away after work to get a drink. She told Kovac that Morris might be on to something that maybe she should plan a destination wedding. They joined Morris and Hope on the other side of the bar and told them there would be no wedding talk. The birthday girl, Sam walked in and everybody toasted drinks.

Neela and Ray continued to have witty banter until both pointed out how awkward it had been since they had shared a kiss earlier. Neela knew they should talk about it but wanted to deal with Gates first. She asked Ray for time to sort out her feelings. She promised Ray he wouldn't have to wait long.

Sam had been depressed ever since she put Alex in a juvenile center to deal with his destructive behavior. Alex had been avoiding her phone calls and emails. Her spirits were lifted when he did call to wish her a happy birthday and to tell her he loved her and missed her.

Sarah came back to tell Gates that she would agree to the paternity test. She asked him if the test proved that she was his biological daughter, could she move in with him indefinitely. He thought that would be a great idea.
