A teenage hockey player with a promising career ahead of him is brought into Med with a possible concussion. After Dr. Choi (Brian Tee) assesses the boy, it's discovered that the teen has a potentially fatal illness -- a brain disease that could kill him after the wrong hit on the rink. Dr. Charles (Oliver Platt) has to physically restrain Dr. Choi after the boy's father and Dr. Choi disagree over relaying medical info to the teen. Even after receiving his diagnosis, the teen decides to continue pursuing his hockey career.

Dr. Rhodes (Colin Donnell) continues his apprenticeship with Dr. Downey (guest star Gregg Henry), the famous surgeon-in-residence at Med. During a risky operation to replace a 9/11 hero's lungs, Downey momentarily takes ill, forcing Rhodes to finish the operation. Later, Rhodes discovers that Downey had been faking his illness, causing Rhodes to rise to the occasion.

Dr. Halstead (Nick Gehlfuss) and Dr. Manning (Torrey DeVitto) have an intense disagreement over the treatment of a patient with congestive heart failure. The disagreement reaches a new level after the patient dies, with Dr. Halstead blaming Dr. Manning for the patient's death. After discovering that he was in error, Dr. Halstead attempts to apologize. When Dr. Manning presses him for an explanation for his behavior, Dr. Halstead expresses his true feelings for her, and the two share a kiss.

Sarah (Rachel DiPillo) debates her future, as she decides where to pursue her residency. Even though she is drawn to Med, those around her are disappointed when she decides to pursue a career in pathology, which will eventually take her away from the emergency department.
