A relatively calm shift becomes chaos as Med treats several victims of a hit-and-run carjacker. Dr. Will Halstead (Nick Gehlfuss) is put in a delicate situation when he learns that the driver, a former felon, committed the crime so he could be sent to jail. The man learned recently he has cancer - and needs the free health care that prison has to offer. Due to doctor-patient confidentiality, Dr. Halstead keeps the carjacker's secret and implores his detective brother, Jay (Jesse Lee Soffer from Chicago P.D.), to do the same.

When a misguided hospital rule gets in the way of a live-saving bone marrow transplant, Goodwin (S. Epatha Merkerson) puts her career on the line by letting the operation proceed as planned. The hospital, not wanting the negative publicity of denying a teenager a second chance at life, lets Goodwin off the hook.

April (Yaya DaCosta) cares for a homeless man who has a peculiar habit of giving away all of his possessions. A consult with Dr. Charles (Oliver Platt) reveals that the man had a stroke, which explains his "giving" nature. Sadly for April, the homeless man's countenance changes radically after another stroke, leaving him stingy and paranoid.

Connor (Colin Donnell) continues his relationship with Dr. Zanetti, and their romance remains an open secret with the rest of the staff.

Will becomes closer to Natalie (Torrey DeVitto) after Natalie's mom (guest star Annie Potts) has a change of heart. Natalie continues to rest in the maternity ward with her new baby, Owen.
