John is getting used to not having his twin brother by his side on the ranch, since he was eliminated the week before. John now recognizes he's representing not only himself, but also his entire family when it comes to getting healthier.

Bob and Jillian want to remind the players just how frustrating the second week on the ranch is. Since the two trainers are double teaming in the gym, they enjoy selecting particular players to pick on.

Dr. H presents the players with folders and in each folder is a challenge. Michael's first challenge is to see how a normal-sized man can carry his excess weight. Bob volunteers to be the guinea pig and is shocked with how painful 300 extra pounds can be on the human body.

The Orange Team's medical task is to come face to face with Jillian, where she will consume just one of the meals they had on a daily basis before they arrived on the ranch. Jillian cannot believe the amounts of fatty foods the mother and son ate in one day.

The players all meet Ali to find out what their challenge will be. She tells the players they will be working on balance. Maria sees the pool of water that the players will fall in if they lose their balance and begins to grow nervous. She is deathly afraid of water and has no idea how to swim.

The players have to walk across a thin balance beam from one side of a pool to the other, while transporting one beach ball at a time. The player who wins the challenge also wins immunity for the week, while the player who gets last place has to face a two-pound disadvantage at the weigh-in. One by one, the teams are losing the battle with their balance.

The race for who will win immunity is neck-and-neck between the Black Team and Red Team. After Andrea falls into the water, Melissa seizes the opportunity with the winning beach ball in her hand. She crosses the finish line and she and her husband celebrate immunity.

One by one, the teams are following in the Red Team's footsteps and completing their challenge. Each team is happy to finish the challenge and not be in last place.

While John and the White Team are the last two competing in the challenge, Maria trips and falls. Immediately, all the players rush to her side as she lies on the floor bleeding. A dazed and confused Maria is rushed to the hospital for further tests; what's worse, the White Team now has a two-pound disadvantage at the weigh-in.

Maria comes back to the ranch with a swollen eye, but physically she's a lot better. Bob has a talk with Michael and understands why he left the ranch to be with his mom at the hospital, but reminds him that he needs to start thinking about himself, instead of Maria.

While Bob is having his talk with Michael, Jillian takes on Maria. Jillian reminds Maria that she is just as strong as all the other mothers on the ranch and she needs to believe in herself. Jillian calls Maria out on her actions and wants her to start overcoming her fears, starting with Maria jumping in the pool.

Andrea has a wake-up call from Dr. H when he shows her the damaging effects that her body fat is having on her organs. After Andrea hears the scary news, Dr. H talks to her father, Darrell, and calls him the sickest man on the ranch. Darrell begins to cry and vows to get healthy.

The players all nervously gather around for the weigh-in. Michael is extremely worried because of the White Team's two-pound disadvantage and the fact that his mother lost a few days of working out because of her injury.

Because the Red Team has immunity, they weigh in first. Lance loses a total of eight pounds and Melissa gains one pound. She can't help but smile, and the rest of the players are shocked. Melissa says she isn't playing a game, but Bob calls her out and says Melissa needs to own up to the fact that she's playing the game to win, not lose weight.

The Purple Team falls below the yellow line and now the rest of the players must decide to send Patti or Stephanie home. Patti pleads to let her daughter stay on the ranch and vows she'll succeed on her own at home, and the players do just that.
