Neela walks through the ER naked and no one notices; she drops hints and no one picks them up. Stepping outside, she realizes -- she's dreaming! She had a hard shift and fell asleep in the shower. Lucien startles her awake, telling her that her next shift in General Surgery started 15 minutes ago. She was looking forward to the night off!

Tony watches Sara play air hockey with her grandfather Hank, when he suddenly collapses. Meanwhile Dr. Pratt is in the middle of a role-playing exercise for conflict resolution. Back in the ER, he has to put up with Frank and Morris arguing while Sam gets a huge bunch of red roses delivered from an admirer whose identity she is not willing to divulge.

Cheryl is brought in on a gurney from an RTA and Zelinsky takes the opportunity to ask Neela if she wants to have lunch with him. She doesn't. In the arcade, Tony decides that Hank should go to the ER just to be safe. Outside the ER, Sam asks Dr. Pratt if he has ever sent roses to a woman. His answer is that the roses are a man's way of saying it wasn't a one night stand. Pratt sees his ex walking past the entrance, but she's too busy to stop and chat.

While Neela and Archie are looking after Cheryl, they find out she's a freshman at the same college where her son is a sophomore. Morris sends Neela, exhausted, to get coffee but before Neela can get it, Sam calls her into another room where Hank is being examined. The mood turns dark as Sara remembers meeting Neela from when her mom died in the hospital.

While looking after Hank, Neela is called into yet another room. When Sam gives Morris a file, Tony takes a moment to ask if she got his special delivery; she thanks him quietly. Elsewhere Dr. Pratt is looking after another patient who isn't making any sense and has multiple broken bones at various stages of healing. Dr. Pratt is later confronted by Frank, who wants to file a grievance against Morris and his aftershave. Pratt is further frustrated by the additional news that Dr. Brenner's flight back from a Milan fashion show has been delayed.

Nicky, Mr. Sinula's son, has been loitering at the reception area and doesn't sound shocked when he is told about his father's injuries. Walking him to his father's room, Dr. Pratt and Sam ask if anyone has been hitting his dad. Nicky suddenly gets defensive and snaps claiming he doesn't know anything. Dr. Pratt doesn't press further but asks how Nicky knows what injuries his father has, considering that Pratt hasn't shared that information.

Hank vomits and his stats drop. Morris alerts the family of the complications. Neela walks Cheryl into surgery and scrubs in for the operation. Back in the ER, Morris has lost his case of energy drinks and blames their disappearance on Frank. Meanwhile Sam confronts Nicky, asking him if he gets physical when he fights with his dad. This causes Nicky to explode with anger.

Dr. Pratt follows Nicky outside and apologizes for Sam's behavior, asking if Nicky knows more than he's letting on. Nicky says when his dad fell in the kitchen he called 911 anonymously through fear of being taken into care if his dad can't look after him anymore; Nicky has no one else. Dr. Pratt decides to run more tests before calling social services. Hank is back from radiology and is sedated and tubed to help him breathe; he is deteriorating further. Morris tells Hank's wife that he needs surgery to remove the blood clot in his bowel but Tony argues, saying drugs can do the job. When Hank's wife opts for surgery, Tony demands to see a hematologist.

Hank's wife finally accuses Tony of being obstinate and egotistical when he keeps pressing for a non-invasive procedure that could waste valuable time. Dr. Pratt tells Nicky that his father's hormone levels are too high, which is making his bones brittle. But with surgery and a week in the hospital he should be fine. Dr Pratt makes a deal with Nicky; if he visits every day and checks in with him, no one will inform social services that he'll be home by himself for the week.

Neela has a jibe at Tony for moving onto Sam so quickly after Julia left him. Tony thinks she's jealous but Neela sets him straight, saying he should think about Sara before inviting another woman into his bed just because he has a phobia of being alone. When Neela presents Hank's wife with the surgical consent form, she suddenly demands they try Tony's option. However, Tony has changed his mind, deciding that surgery will give him the best outcome.

In Hank's surgery, Neela has to be asked three times for the solution to a procedure â�" exhaustion has taken hold. It's then that Hank starts to bleed. Dr. Pratt walks Nicky and his dad to another part of the hospital, telling them that the excess calcium has made Mr. Sinula seem deranged. Back at reception, Dr. Pratt puts his new conflict resolution skills into practice because Frank and Morris are at each others' throats. Using his new "I statement" tools, Dr. Pratt finds out that the argument stems from Morris excluding Frank from his party invite. The humble pie is dished up as the men share a hug.

Hank comes out of surgery in good shape and Neela is accosted straight away by Zelinsky. He tells her it was his decision to leave surgery because he doesn't want to end up alone, like her. Sam leaves with Tony carrying her roses. They pass Morris, who then realizes the couple's secret. Zelinsky's comments have hurt Neela. While in the staff room with Lucien, she explains she hates being too busy to date, and is too lonely to be happy in her career and that nobody even said happy birthday to her. In order to remind Neela of the miracles she performs daily, Lucien takes her to see all the patients she operated on that day who are now in recovery. Even Sara says thanks.

Dr. Pratt bumps into his ex again and finds out that her recent colonoscopy results showed suspicious tissue. Things aren't good. She confides in him her choice to not undergo preventative surgery, as her family's history of cancer means that if one type doesn't get her, another will. Dr. Pratt offers to take care of her but she refuses, saying he made his choice when they split up. In the hospital, Tony mends bridges by asking Hank's wife to stay with them while he is in the hospital. Just before she leaves, Tony wishes Neela a happy birthday.
