Drs. Pratt and Brinner work to save a hit-and-run victim's life. Meanwhile, Archie and Tony are up on the helipad with Sam getting Daniel and his girlfriend Rebecca - who have been injured in a car crash - from the paramedic helicopter. As the paramedics take Rebecca down off the roof into the hospital, Daniel, who is conscious, takes the opportunity to sneak a gun from the helicopter into his pocket. Meanwhile Abby's at home with Joe, handing him to Luka as she heads out to work.

Drs. Pratt and Brinner trade insults at reception. Pratt's trying to wrap things up for the day so he can get to a personal appointment, but this means leaving Larry and Laverne in the hands of Dr. Brinner. The police ask about the hit-and-run patient as Daniel and Rebecca are wheeled past. Daniel looks at them nervously. Abby follows Pratt into the locker room, curious about the board's decision. Pratt tells her they haven't decided yet, but that she looks good, although she will have to stay in limbo until a decision is made.

Larry argues with Archie over whether Rebecca has a collapsed lung and needs a procedure to treat it. Larry wants to rush ahead, pointing out he'll be a resident in two months and knows what he's doing. But Archie tells him to wait for the x-rays. Rebecca tells them they were driving fast as they were on their way to see Daniel's mom, who is dying from cancer. Archie checks on Daniel, who is asking to see Rebecca, but he has to get a few stitches in the gash on his arm before they will let him go anywhere.

Hesitantly, Dr. Pratt goes to see his ex as she prepares for her operation. She gives him a cold reception. Meanwhile in the ER, Abby deals with a guy who has been drinking colloidal silver as a natural antibiotic, but the mineral deposits have turned his skin blue. Archie rushes to stop Larry crashing ahead with the chest tube in Rebecca's lung without the test results and averts a potential disaster. Abby tells Neela that Luka moved out and the pair arrange to talk about it over dinner. Daniel makes a nervous call to someone telling them he and Rebecca will meet them in an hour. As he talks, he notices Frank watching the news about a diamond robbery in the city.

Back by Rebecca's side, Daniel tells her about the robbery on the news. He and Rebecca seem to be having a private conversation when Archie asks them if they live nearby. A policeman comes in to inquire about the hit-and-run kid and once again Daniel acts nervous. Rebecca tells him to go see his mom, and Archie tells him to go as well before he leaves. Daniel heads out, but with cops milling outside on alert after the robbery, he heads back inside to Rebecca's room - just as her heart stops.

Dr. Tony Gate and Sam attend to a boy who fell from a unicycle and broke his leg. Jumping into action, Neela tries massaging Rebecca's heart, and after 45 minutes she wants to call time but Daniel won't let her. Archie suggests taking her to the OR, but Neela doesn't want to send her up with her massive injuries. After hearing the news of Rebecca's plight, Daniel pulls out the gun and demands that they fix her. He closes all the blinds and barricades the doors, locking them in the trauma room.

Neela dials for supplies with Daniel pointing the gun at her. Archie tries to ease the stress but it doesn't help. Outside, the unicycle boy's dad has arrived and he isn't happy. Meanwhile, Dr. Pratt has been sitting alone outside while his ex has her operation. Back at her bedside, she tells him the procedure was more invasive than they thought. She admits she's touched that he waited through the whole operation. Gregg tells her he's sorry for being hardheaded and they vow to work together to fix things.

Daniel complains a student doctor is operating on Rebecca but there's nothing anyone can do as he's holding them hostage without the rest of the ER knowing. Then her heart starts beating again and things look up. Outside, Tony has the x-rays for the unicycle boy and takes him into the trauma room next to Rebecca's room. When he needs some equipment from next door he is amazed to see an operation underway. A sudden spurt of blood sends Daniel into a panic but Archie manages to point Tony's attention to Daniel's gun, alerting Tony to the danger.

As Neela works on Rebecca, Archie distracts Daniel by showing him the stitch that will save her. He hopes Tony can surprise Daniel and tackle the gun from him. Tony uses a metal tray to hit the gunman in the head. After a scuffle, Daniel regains control of the weapon and fires a shot through the door into the other trauma room, prompting an evacuation of the whole ER.

Cops pour into the ER as they evacuate the building. Everyone is removed except for the boy with the broken leg, who has been forgotten. Outside cops and EMTs flood through the hospital getting everyone out when Tony Gates appears, carrying the unicycle boy out of the building to safety. The cops try phoning the trauma room as Neela orders more blood transfusions and sends Daniel to get it while police marksmen stand in guard. The cops call again and talk to Daniel, but he slams the phone down on them.

Archie sucks up his fears and confronts Daniel with the fact that Rebecca is beyond help and they should let her go as she will be brain dead now if she were to wake up. Petrified, Neela backs up Archie as he confronts Daniel, who is beginning to break down. Archie tells Daniel to say goodbye to Rebecca before they stop massaging her heart. Daniel breaks down in tears and accepts Rebecca's fate. Archie asks for the gun and after a long pause, Daniel does the right thing. As soon as the gun is in Archie's hand a marksman shoots Daniel dead.
