Abby and Luka, at home in the morning, argue over Abby's infidelity and the state of their marriage. Gates walks into the ER with two dozen donuts and offers one to Sam, who immediately tenses thinking people will start to suspect something between her and Gates. Pratt and Sam meet an approaching ambulance. The victim is covered in gasoline from an erupted tank. A cigarette is tossed and it lights the man on fire.

Neela, who has become friendly with a patient, Cheryl, discusses her dating life, or lack thereof at the moment. The gasoline patient is in the ER where the doctors are attending to his burns sustained from the fireball outside. A 19 year old is brought into the ER after being mugged on the platform of the L. Abby takes the patient; Sam, working with Abby, is clearly annoyed and still has unresolved issues towards her. Morris talks to Breener hoping for info on his new lady, whom Breener implies works with them.

A woman, pregnant with her 13th child, is wheeled into the ER after getting into a fender bender. Abby and Breener find a cyst on the 19 year old's neck and then head into the trauma room to see the pregnant woman. Sam calls surgery in to to consult on the cyst.

Luka is on a job interview at a elderly care facility. He takes a moment to walk through the facility, meeting a older man named Walter, who is dying of cancer and wants a friend to play cards with. Walter asks Luka why he would want to work in a place like that rather than continue working in the ER. Back in the ER, Dubenko works on a dislocated arm while Dr.'s Wexler and Breener look on. The sexual tension between the three of them is papable.

Neela and Abby continue to look at the young man who has a potentially infected cyst on his neck. At the nurse's station, Breener shows the docs how to get lab reports on their phones, Pratt fixes the board, and Gates passes on some patient information to Sam. Anspaugh talks to the staff and asks about Breener, who is revealed to be his nephew.

Abby approaches Sam to discuss the tension between them. Sam rejects the conversation and goes about her work. Luka continues to play cards with Walter at the home. Luka tells him about his recent travels to Croatia to see his family and be with his father on his deathbed. Back in the ER, Neela is working on the 19 year old's cyst when Dr. Breener calls in on speaker telling the room the boy has cancer, not realizing everyone can hear him. The young man, visibly upset, is sedated by Neela.

After arguing with Neela outside the trauma room about her choice to give him a sedative, Abby decides how she will tell the patient about the cancer. Walter asks Luka why he went back to Croatia. Luka shares with Walter that his father's death taught him he can't fix everything. Dubenko and Wexler discuss cancelling their upcoming weekend vacation to accommodate a dive Wexler already had planned with some guy friends of hers.

Morris gives Pratt advice on his situation with ex-girlfriend Bettina. Pratt, trying to respect her wishes, doesn't want to overstep his boundries. The pregnant woman takes a turn for the worse and goes into labor in the ER and starts to bleed out on the gurney. Abby makes a decision that they need to deliver in the ER or risk losing both mom and baby.

Dr. Coburn arrives in the ER to help Abby with the baby and mom. Abby pulled off a successful delivery and Breener sings her praises to Neela, who chews him out for his lack of sensitivity with the cancer patient. He apologies to Neela and walks away.

Dubenko, insecure over Wexler's friendships with other men, accuses her of being with several people and having an inability to attach to one person. Offended, she walks out. Sam approaches Abby outside to apologize for being judgemental because she grew up around people with a lot of similar problems. They call a truce.

Abby approaches Anspaugh for another chance at interviewing for the attending position. Neela returns to see her patient from earlier, Cheryl, to tell her that she has a post-op infection that requires another surgery. Neela, upset, is having trouble telling Cheryl about the infection because they have become friends, but Cheryl is optimistic.

Morris looks on and admires the family of 13 kids. Dubenko opens the gift that Wexler got him before he yelled at her. Gates opens his locker to find a note asking him to meet on the roof. Pratt brings cupcakes, per Morris' suggestion, to Bettina's brownstone. Abby tells the 19 year old of his diagnosis. Luka walks the streets of Chicago thinking of the future and eventually returns home to Abby.

Luka takes the job at the elderly home. He tells Abby that he wants to tell her he is fine, but too much has happened and he must move out.
