Abby had five minutes left in her shift and got called in for a transport on the helicopter. Nobody would take the patient for her, even though it was Thanksgiving and she wanted to be home with Joe. Jane, the intern, told her that Abby had to take the patient, just then, a young girl, Lulu, came in complaining of stomach pains and vomited all over Jane. Abby blamed karma for Jane's mess.

Abby was forced to have Kovac finish the Thanksgiving dinner at home while she went on her first helicopter ride for her transport patient. Abby arrived to the county hospital to find an 88-year-old heart attack patient who couldn't even breathe on her own. Abby alarmed the staff when she informed them the woman was too sick to be transported back on the helicopter.

Gates paged Neela to the ER. He managed to get her alone just to tell her how hot she looked in her scrubs. He told her he couldn't even concentrate on his job because she was on his mind so much. She laughed it off and went back to the surgery.

Abby was convinced that their patient was going to die on the way back to County General. She argued with one of the other medics who thought they should take the risk and fly her back. Abby thought the woman should die with her family by her side. They ended up flying the patient back anyway.

Alex was volunteering for Thanksgiving, mostly because his mom made him. While Sam did rounds, Alex met Lulu. She had to go in for an x-ray and volunteered Alex to be there by her side because her own parents had not shown up.

Abby's patient was going into cardiac arrest while on the helicopter. She began frantically trying to save her life, while lecturing the medics that the patient shouldn't have been flown on a helicopter to begin with. The woman ended up dying on their way to Country General.

The transport responded to a car accident on a remote highway. The medics wanted to avoid the call, but Abby radioed in that they would make the stop. When they arrived, they found an overturned bus with passengers fleeing. There was nowhere for the helicopter to land and no room to carry all the passengers. Abby had to be lowered down by rope.

Abby was overwhelmed with how many injured people there were pouring out of the bus. As the bus was dangling over a cliff and Abby noticed a young boy still inside screaming for help. She climbed inside and found the boy with his mother. Abby tried to free the woman but the seat that was on top of her was too heavy. As the bus began to shake, Abby grabbed the little boy and escaped the bus.

Abby convinced the woman's son to help her out on the field. She told him that his mother was going to be fine, especially when the fire department arrives. Abby helped out all the injured people who had escaped the bus. When the fire department did arrive, they told Abby that it was too dangerous to go back on to the bus and rescue Milo's mother, Melanie.

Abby defied the firemen and went back into the bus to help out Melanie. She was bleeding heavily and Abby knew she didn't have a lot of time. One of the medics arrived with a device to help lift the seat off of Melanie.

Crenshaw wanted to know information about Jane. Ray called him out by telling Crenshaw it was obvious he was interested in her. Crenshaw just played it down and called Ray a juvenile for even suggesting it.

Abby and the medic were able to rescue Melanie and get her off the bus. As soon as they jumped off the bus, it fell over the edge and into a fiery crash. They were allowed to transport survivors to County General for treatment.

Gates was waiting for Neela to finish her shift at the hospital. He explained to her that he had the place to himself and wanted her to come home with him after work. She took him up on the offer and went to his place after work. She told Gates she wanted to take things slow, but immediately started making out with him.

Weaver was taking interest in her news producer, Courtney. Pratt walked in on the two sharing dinner in the break room. Courtney excused herself and Pratt had some questions for Weaver regarding his brother. He was wondering if his brother could really be gay or just going through a stage. She told him that he was probably gay, but too afraid to admit it to Pratt and said Chazz's life will probably be a lot better if Pratt accepted him as a gay man.

Everybody was giving Abby a hard time for not staying in the ER. She was completely exhausted and just wanted to go home. Kovac and Joe surprised Abby at work with a visit. Kovac had brought Thanksgiving dinner for everybody who was working the ER that night.

Neela and Gates made love and had a surprise awakening. Gates roommate and daughter came home early and found them. Neela was embarrassed and Gates felt it was appropriate to introduce everybody.

After such an exhausting day, Abby thought it was appropriate to tell Kovac all the things she would like him to remember if she was to die. Kovac didn't know what sparked this kind of talk and Abby told him she didn't want to spoil Thanksgiving, and she would tell him tomorrow. Then Pratt led the rest of the table into a toast to Kovac for the wonderful dinner.
