Catherine tells Will that she's Esme's mother to avoid coming clean about her relationship with Bickman. She needs to make a decision about the two men in her life. In other news, Esme is hired at Will's restaurant to help out. It isn't long before she discovers the truth about Delilah. She embarks on a bit of sabotage to get rid of the hostess from hell. Her plan works to perfection. Will finally has the grounds to fire Delilah.

A mother is at the end of her rope when her daughter, Lily, displays signs of meningitis and fails to be diagnosed by several doctors. The young girl has a fear of water. This leads Catherine to believe she was bitten by a bat. She has an advanced stage as rabies. There's no cure. Lily's mother begs Catherine to no give up on her daughter. They have one week to save her.

Catherine, who goes off her meds to be at the top of her game, researches something called the Milwaukee Protocol which involves putting Lily into a medically-induced coma. The patient may not wake up. Lily's mom gives the go head to try it. Farrah suspects something is amiss with Catherine, who pins her to a wall when confronted. It's an unsettling moment. Moments later, Catherine locks herself in her office and downs her meds.

Owen admits a patient who is convinced he is dead. Doomed to enduring the advanced onset of the hallucination, he reluctantly agrees to some tests that reveal he has Cotard's Syndrome. The patient believes his flesh is rotting. The case fascinates Owen.

Bickman finds Owen's patient bleeding out after a suicide attempt. Once the walking dead guy recovers, Owen performs an aggressive procedure on him. It has the guy feeling he's not quite as dead as he used to be. The hallucinations he'd been having of looking like a rotting corpse are gone.

Catherine tells Farrah that she took amphetamines to help her through Lily's case. She thanks her for understanding. As for Lily, she develops a fever. Bickman suggests they freeze it out. Soon Lily's temperature stabilizes. Everyone standing by Lily's side is overjoyed when she finally opens her eyes. Catherine and Bick are also relieved as they share a kiss in the elevator. Catherine later returns her engagement ring to Will.
